Where ominous bottles come from
Ominous bottles drop from vaults in trial chambers and from pillager raid captains. Since Java 1.21, killing a raid captain no longer gives you Bad Omen directly — the captain drops this bottle instead, so you choose when to trigger the omen instead of getting raided by accident.
Bottles come in levels I through V. Higher-level bottles mean a stronger Bad Omen, which upgrades the difficulty — and the loot — of the ominous trial or raid you trigger.
Vaults and ominous vaults in trial chambers drop them.
Pillager captains (outposts, patrols) drop them instead of applying Bad Omen.

What to do with Bad Omen
Drink the bottle and you carry Bad Omen for up to 100 minutes. Walk near a trial spawner and it becomes Trial Omen: nearby spawners turn ominous, mobs get tougher, and defeated ominous spawners drop ominous trial keys — the only keys that open ominous vaults, which are the sole source of the Wind Burst enchantment.
Walk into a village instead and Bad Omen becomes Raid Omen, starting a pillager raid. Win the raid for the Hero of the Village effect and its trading discounts. If you change your mind, drinking milk clears the omen before it converts.
